Below are the various forms used by St. Walter School. Most of the forms are in a PDF format and can be downloaded and printed for your convenience. Note: If you encounter problems, you may need to download the current version of Adobe to open these files.

Field Trip Form
Field Trip Form	Permission to attend field trips sponsored by St. Walter School must be given in writing on the *Field Trip Authorization Form*. No other form of permission, either verbal or in writing will be accepted.
School Apparel Forms We will be not accepting new orders until next September NOTE: Next Try-on Sale - April 28
PE Wear Uniform Sweatshirts Grey Sport Shirts Hoodies Bags	For students in grades 5 through 8, we offer a grey polo shirts with St. Walter School embroidered on the front. As an option, this shirt may be worn in place of the white blouse or white polo. Orders are processed the 15th of each month.
	A light grey sweatshirt is offered as an option to be worn with the uniform or on spirit days. This sweatshirt may not be worn for P.E. Orders are processed the 15th of each month.
	Students in grades 1 through 8 wear an official school uniform for gym class. The uniform options are shirt, short, sweat pant, and/or sweatshirt. The P.E. wear offers a newly designed logo with our school’s mascot, the Spartan. Note: Previous St. Walter School P.E. wear is acceptable. Orders are processed the 15th of each month.
	A hooded sweatshirt is available and can be worn as outerwear or on spirit days. It is not part of our official uniform and cannot be worn during class. For safety reason, it cannot be worn during P.E. classes. Orders are processed the 15th of each month.
	Canvas or nylon sport bags are offered as an option for toting P.E. clothes. Orders are processed the 15th of each month.
Jackets & Adult Sweatshirts	A wide array of sport shirts, jackets and sweatshirts are available for the entire family. All items are custom embroidered with the school logo. Names can be embroidered on the jackets. The line also includes parent and grandparent sweatshirts. Can be ordered only on the try-on sale days.
Flannel Pant Order Form	A blue plaid flannel pant with St. Walter School logo running down the side leg is available and can be worn on spirit, out-of-uniform and PJ days! Can be ordered only on the try-on sale days.

Shopping Certificate Disclaimer Form	This form must be completed before the release of certificates
Shopping Certificate Order Form	Form to order the certificates. Pay face value, receive them one week later and get a deduction on your next tuition statement! The amount deducted depends on the amount you buy.
